We need to know about your pet!
If you are interested in using our daycare or boarding services, all dogs coming to Inn the Doghouse for the first time must complete a Trial Day. This is to establish that this is the right environment for them. During Trial Day, our trained staff monitors their temperament and behavior. Once your dog passes their Trial Day, please call to schedule an appointment or for Daycare, Boarding, and Grooming Services.
If you would like to set up a Trial Day for your dog or have any questions or concerns, please feel free to call us at (440-780-4299 anytime, Monday-Friday from 7 am-7 pm, and one of our staff members would be happy to assist you.
In addition to the forms below, written proof of current vaccinations is required for all dogs and cats (dogs: DHLPP, Rabies, Bordatella, | cats: FVRCP, Rabies), and dogs are required to present an Annual Clean Fecal Test from your veterinarian. Most vets will not require a visit for this, only a sample. Please print and fill out the following forms; then, either fax them to us or bring them in when you plan to visit. If you have any questions, please call us for assistance.
You can download, review, and complete these forms (PDF format) and bring them with you to the Trial Day.
You can also fill out the forms when you visit us. In addition, all dogs must be at least 4 months old, and be spayed or neutered if over 7 months old. For their safety, pets must be on a leash or in a carrier when entering and leaving Inn The Doghouse. Thank you for your understanding.
